...I've seen a judge get a javelin in the leg, this is going back a while, maybe the 80's at some point.
I seem to recall an olympian knocking himself senseless after an (amusing) fail at a hammer toss, I think he knocked his own jaw out of place.
As for the shot put, I've not seen that yet, but there is always hope.
